【摘要】 目的 评估胆道125I支架治疗恶性胆道梗阻的有效性和安全性。方法 回顾性分析2015年1月至2019年12月收治的恶性胆道梗阻患者临床资料。其中接受经皮胆道125I粒子支架植入治疗40例，单纯胆道金属裸支架植入75例。比较两组近期疗效及并发症、胆道支架通畅时间、生存时间等。结果 两组患者手术成功率100%，无手术相关死亡发生，两组患者的术后近期疗效及并发症的比较差异无统计学意义（P＞0.05），胆道通畅时间分别为（8.93±3.28）和（6.59±3.14）个月，术后平均生存时间分别为（12.12±6.28）和（9.02±4.12）个月，差异有统计学意义（P＜0.05）。经单因素及多因素分析显示，治疗方式及肝内病灶是影响患者生存时间的独立影响因素。 结论 胆道125I粒子支架作为恶性胆道梗阻患者的姑息治疗方案安全有效可行，能延长支架通畅时间及患者生存期。
