摘要:
【摘要】 目的 分析经肾动脉移植骨髓间充质干细胞(BMSC)对慢性肾病(CKD)大鼠肾脏炎症的影响。方法 取37只雄性Sprague- Dawley(SD)大鼠,其中2只4周龄用作体外分离BMSC,8只为正常对照组(N组),27只制作CKD模型。建模中3只死亡,24只建模成功后,随机分为CKD组(n=8)、经肾动脉移植BMSC组(A组,n=8)和经尾静脉移植BMSC组(V组, n=8)。BMSC移植后7、14 d,检测各组大鼠肾功能指标,制作肾脏病理切片,苏木精- 伊红(HE)染色观察肾脏病理形态学变化,免疫组化染色分析肾组织炎性细胞表达。结果 CKD组、A组、V组与N组相比,BMSC移植后血肌酐、血尿素氮和24 h尿蛋白均显著升高(P<0.01)。移植后7、14 d,V组、A组血肌酐均较CKD组降低(P<0.01),A组在7 d时下降更为显著(P<0.01);移植后7 d,V组、A组血尿素氮较CKD组均显著下降(P<0.01),A组变化更为显著(P<0.01),移植后14 d,V组、A组较CKD组稍降低,A组变化更为显著(P<0.01);移植后7、14 d,V组、A组24 h尿蛋白较CKD组均显著降低(P<0.01),A组下降更为显著(P<0.01)。HE染色病理结果显示A组、V组炎性浸润均较CKD组减轻,A组仅有少量肾间质炎性细胞浸润,萎缩肾小球数目减少,肾小管扩张情况有不同程度改善。免疫组化结果显示CKD组大鼠CD3、CD20呈强阳性或阳性表达,A组、V组大鼠肾间质CD3、CD20表达较CKD组减轻,但A组较V组减轻更为明显。 结论 BMSC通过抑制免疫介导性炎症修复受损肾脏组织。经肾动脉移植效果优于经尾静脉移植。
