摘要:
【摘要】 目的 设计一种可携带125I粒子的鼻饲营养管,用于治疗上消化道恶性阻塞性病变,以达到鼻饲营养和腔道内近距离放疗的双重效果。方法 将125I放射性粒子置入3 F导管内并两端封闭制成粒子链,使用捆绑技术将粒子链固定于鼻饲管上,并在DSA透视下置入可携125I粒子链的鼻饲营养管,其中粒子链段跨越食管闭塞或狭窄段行近距离放疗。6例晚期食管癌使用该项技术,记录技术成功率,操作时间,并发症。术后2个月评价Karnofsky评分,疼痛视觉模拟评分(VAS),Neuhaus吞咽困难分级,对比剂通畅情况,肿瘤局部情况。结果 可携带125I放射性粒子链的鼻饲营养管制作简单,技术均获成功,平均操作时间15.5 min,无出血、感染等严重并发症。术后2个月Karnofsky评分、VAS评分、Neuhaus吞咽困难分级均较术前明显改善(P<0.01),对比剂通过闭塞狭窄区顺利,肿瘤评价均为部分缓解、局部控制。随访2~6个月,再发梗阻1例。 结论 可携带125I粒子的鼻饲营养管制作简单易行,可同时实现胃肠营养和近距离放疗,为食管恶性肿瘤所致阻塞性病变的治疗开辟了新的途径。
