【摘要】 目的 探讨双针成角顺序活检-消融术对肺结节诊疗的可行性和安全性。方法 回顾性收集143例行肺结节热消融术的患者资料,其中A组89例100个结节行顺序活检-消融,B组54例58个结节行直接消融或活检诊断后消融。比较两组手术成功率,手术时间,即时和延迟并发症的发生率;评价双针成角顺序活检-消融术在肺结节诊治中的可行性和安全性。结果 两组穿刺活检和消融技术成功率均为100%。A组的手术耗时为(55.79±32.09) min,B组为(54.76±27.75) min,手术耗时差异无统计学意义(P=0.832)。A组气胸即时发生率19%(19/100),B组为20.7%(12/58),胸腔积液发生率分别为6%(6/100)和8.6%(5/58),二者差异无统计学意义(P=0.533)。延迟气胸发生率分别为5%(5/100)和8.6%(5/58);即时和延迟气胸的置管率分别为6%(6/100)和3.4%(2/58),上述比较,两组差异均无统计学意义(P均>0.05)。两组各3例患者出现术中咯血,无空气栓塞等其他致死性并发症。结论 活检针和消融针的成角分布顺序活检-消融术对肺结节诊疗是一种安全可行的方法。
