【摘要】 目的 探讨CT引导下不同穿刺路径对肺底胸膜下小结节(≤1.5 cm)穿刺活检成功率、调针次数、手术时间、并发症发生率的影响。方法 回顾性分析2013年6月—2016年12月CT引导下经皮肺底胸膜下小结节穿刺活检的临床及影像学资料,根据穿刺路径分为最短路径(A组)和长路径(B组)进针两组,分别记录两组患者性别、年龄、肺结节大小、调针次数、手术时间、穿刺成功率、气胸及出血发生率,及B组的针-胸膜角度、针道长度;分析比较两种路径的活检成功率、调针次数、手术时间及并发症发生率等。结果 B组单次穿刺成功率稍大于A组,但差异未达统计学意义(P=0.80)。A组调针次数及穿刺时间明显大于B组(P<0.05)。结论 肺底胸膜下小结节(≤1.5 cm)一般采取斜行进针,在保证有效性和安全性的情况下,尽量选择较大针-胸膜角度和/或较短针道长度,可有效提高穿刺成功率,减少调针次数和手术时间。
