【摘要】 目的 对比分析改良手动抽吸血栓清除(MAT)、经皮机械性血栓清除(PMT)及单纯导管接触溶栓(CDT)治疗急性髂股静脉血栓形成的临床效果及安全性。方法 回顾性收集2016年2月至12月经下肢静脉彩色超声或造影确诊的60例急性髂股静脉血栓形成患者临床资料。根据治疗方案不同,分为改良MAT辅助CDT组(A组,n=18)、AngioJet PMT辅助CDT组(B组,n=12),单纯CDT组(C组,n=30)。对三组患者术后血栓清除率、血栓清除过程失血量、CDT时间、尿激酶(UK)用量、患肢消肿程度、住院时间及相关并发症进行评估。结果 术后A、B、C组分别有17例(94.4%)、11例(91.7%)、26例(87.7%)获临床成功(血栓清除达Ⅱ级以上),三组间差异无统计学意义(P>0.05)。A、B组血栓抽吸中失血量分别为(99.8±11.4) mL、(117.6±12.3) mL(P=0.001);CDT时间分别为(2.9±1.5) d、(2.8±1.6) d(P=0.922),均较C组(7.1±1.4) d显著降低(P<0.001);UK用量分别为(166.7±93.9)万U、(121.7±115.2)万U(P=0.918),均较C组(325.8±106.8)万U显著减少(P<0.001);24 h患肢消肿程度差异无统计学意义(P>0.05),但均优于C组(P<0.05);住院时间分别为(6.1±1.4) d、(5.9±1.7) d(P>0.05),均较C组(11.0±1.8) d显著缩短(P<0.001)。三组患者髂静脉支架植入枚数、手术相关并发症、血栓后综合征发生率及严重度差异均无统计学意义(P>0.05)。结论 改良MAT辅助CDT治疗急性髂股静脉血栓形成原理与AngioJet PMT辅助CDT治疗类似,临床疗效相似且安全,可降低手术费用;与单纯CDT治疗相比,可有效缩短溶栓时间、减少UK用量、减轻患肢肿胀、缩短住院时间,值得推广。
