【摘要】 目的 探讨血型与急性下肢深静脉血栓形成（DVT）预后的相关性。方法 回顾性分析2017年1月至2019年6月经川北医学院附属医院血管外科收治的急性下肢DVT患者的临床数据及1年期随访资料。按血型分成O型血组和非O型血组，对比两组患者住院期间患肢消肿率、血栓清除率、尿激酶用量、溶栓时间、住院时间、并发症发生率及出院后1年血栓复发率、血栓形成后综合征（PTS）发生率及Villalta评分，评估血型对DVT预后的影响。单因素和多因素logistic回归分析发生PTS的危险因素。结果 非O型血组DVT患者的PTS发生率、Villalta评分和治疗后的静脉通畅度得分均较O型血组高，且差异有统计学意义（P＜0.05）。而患肢消肿率、血栓清除率、住院时间、并发症发生率等差异无统计学意义（P＞0.05）。logistic回归分析结果显示，O型或非O血型（OR=4.337）、治疗后D-二聚体水平（OR=1.116）、合并肿瘤（OR=6.679）、治疗方式（OR=0.230）、血栓清除率（OR=0.900）和血栓复发（OR =97.628）是急性下肢DVT患者1年内发生PTS的独立危险因素。结论 非O型血是急性下肢DVT患者发生PTS的独立危险因素，在治疗急性下肢DVT患者时，应考虑到血型对预后的影响，从而实施个体化的最优治疗。
