【摘要】 目的 研究导管接触溶栓（CDT）治疗急性下肢深静脉血栓形成（LEDVT）过程中血浆D-二聚体（D- D）和纤维蛋白原（FIB）变化，分析其意义。方法 选取2016年12月至 2017年12月采用CDT术治疗的18例急性LEDVT患者，治疗中每隔8 h检测患者血浆D- D、FIB浓度。计算每例患者D- D峰值、D- D上升速度、FIB下降速度及D- D增高时间，统计学分析D- D、FIB变化规律，以及D- D峰值、D- D上升速度、FIB下降速度及D- D增高时间之间相关性和线性关系。结果 CDT术后18例患者中13例（72%）疗效评价为优或良，无严重出血并发症发生。CDT治疗中尿激酶微泵速度不变情况下，D- D先上升后下降，FIB持续下降；尿激酶微泵速度减半后，D- D下降速度变缓，FIB逐渐上升。统计学分析显示，D- D峰值与D- D上升速度呈正相关（Pearson比0.88，P＜0.01），D- D峰值与D- D增高时间呈正相关（Pearson比0.80，P＜0.01），D- D上升速度与D- D增高时间呈正相关（Pearson比0.62，P=0.01），D- D上升速度与FIB下降速度呈正相关（Pearson比0.49，P=0.04），D- D峰值与FIB下降速度呈正相关（Pearson比0.76，P＜0.01），D- D增高时间与FIB下降速度间无相关性。结论 急性LEDVT患者CDT治疗过程中血浆D- D、FIB会产生规律性变化，监测和分析D- D、FIB变化规律，有助于提高CDT疗效，减少并发症。
