【摘要】 目的 研究导管接触溶栓(CDT)治疗急性下肢深静脉血栓形成(LEDVT)过程中血浆D-二聚体(D- D)和纤维蛋白原(FIB)变化,分析其意义。方法 选取2016年12月至 2017年12月采用CDT术治疗的18例急性LEDVT患者,治疗中每隔8 h检测患者血浆D- D、FIB浓度。计算每例患者D- D峰值、D- D上升速度、FIB下降速度及D- D增高时间,统计学分析D- D、FIB变化规律,以及D- D峰值、D- D上升速度、FIB下降速度及D- D增高时间之间相关性和线性关系。结果 CDT术后18例患者中13例(72%)疗效评价为优或良,无严重出血并发症发生。CDT治疗中尿激酶微泵速度不变情况下,D- D先上升后下降,FIB持续下降;尿激酶微泵速度减半后,D- D下降速度变缓,FIB逐渐上升。统计学分析显示,D- D峰值与D- D上升速度呈正相关(Pearson比0.88,P<0.01),D- D峰值与D- D增高时间呈正相关(Pearson比0.80,P<0.01),D- D上升速度与D- D增高时间呈正相关(Pearson比0.62,P=0.01),D- D上升速度与FIB下降速度呈正相关(Pearson比0.49,P=0.04),D- D峰值与FIB下降速度呈正相关(Pearson比0.76,P<0.01),D- D增高时间与FIB下降速度间无相关性。结论 急性LEDVT患者CDT治疗过程中血浆D- D、FIB会产生规律性变化,监测和分析D- D、FIB变化规律,有助于提高CDT疗效,减少并发症。
