【摘要】 目的 评价基底动脉(BA)动脉瘤形成与患者年龄、BA分叉形态学及血流动力学应力的关系。方法 连续收集石家庄市第一医院收治的195例患者脑部DSA造影资料,其中BA动脉瘤患者56例(研究组),无动脉瘤患者139例(正常对照组)。检测两组患者BA动脉分叉角度、BA直径、分叉顶端血流动力学应力以及外侧角(LA)比值、动脉直径(DA)比值。多元线性回归分析和受试者特征曲线(ROC)评价动脉分叉形态学参数对动脉瘤形成的影响,检测分叉顶角角度改变后血流动力学应力变化。结果 40~70岁女性更易形成BA动脉瘤,分叉顶角与患者年龄呈显著正相关(P<0.000 1);研究组分叉顶角显著大于正常对照组(P<0.000 1),BA直径、两侧大脑后动脉(PCA)直径、DA比值与对照组差异无统计学意义(P>0.05)。ROC分析显示BA分叉顶角曲线下面积(AUC)为0.924,最佳截断点为117.9°。动脉瘤多朝向小的外侧角。与支架植入前相比,支架植入后分叉顶角角度和血流动力学应力均减小。结论 BA动脉瘤形成与患者年龄、较宽的动脉分叉顶角及相应血流动力学应力均呈显著相关性,BA分叉顶角对预测动脉瘤形成有重要参考价值。
