【摘要】 目的 探讨血型与急性下肢深静脉血栓形成(DVT)预后的相关性。方法 回顾性分析2017年1月至2019年6月经川北医学院附属医院血管外科收治的急性下肢DVT患者的临床数据及1年期随访资料。按血型分成O型血组和非O型血组,对比两组患者住院期间患肢消肿率、血栓清除率、尿激酶用量、溶栓时间、住院时间、并发症发生率及出院后1年血栓复发率、血栓形成后综合征(PTS)发生率及Villalta评分,评估血型对DVT预后的影响。单因素和多因素logistic回归分析发生PTS的危险因素。结果 非O型血组DVT患者的PTS发生率、Villalta评分和治疗后的静脉通畅度得分均较O型血组高,且差异有统计学意义(P<0.05)。而患肢消肿率、血栓清除率、住院时间、并发症发生率等差异无统计学意义(P>0.05)。logistic回归分析结果显示,O型或非O血型(OR=4.337)、治疗后D-二聚体水平(OR=1.116)、合并肿瘤(OR=6.679)、治疗方式(OR=0.230)、血栓清除率(OR=0.900)和血栓复发(OR =97.628)是急性下肢DVT患者1年内发生PTS的独立危险因素。结论 非O型血是急性下肢DVT患者发生PTS的独立危险因素,在治疗急性下肢DVT患者时,应考虑到血型对预后的影响,从而实施个体化的最优治疗。
