【摘要】 目的 探究3D打印技术在颅内动脉瘤介入治疗中的应用价值。方法 选取徐州医科大学附属医院介入放射科2021年10月至2022年3月收治的经脑动脉造影术提示颅内动脉瘤的患者40例。将患者随机分为试验组20例,对照组20例。试验组术者利用3D- DSA数据制作动脉瘤模型,通过术前模拟,术中辅助微导管塑形。对照组术者通过术中观察3D- DSA图像,凭借临床经验塑形微导管。比较两组术中微导管体外反复塑形的次数、微导管末次塑形后从指引导管至到达动脉瘤腔的时间、首次塑形微导管后微导管抵达动脉瘤腔的成功率、术中单根微导管塑形综合评分以及术后2个月改良Rankin量表(mRs)评分。结果 试验组微导管体外反复塑形的次数为(1.30±0.47)次,微导管末次塑形后从指引导管至到达动脉瘤腔的时间为35.00(20.00,67.50)s,首次塑形微导管后微导管抵达动脉瘤腔的成功率为70%(14/20),术中微导管塑形综合评分为(3.15±0.99)分。对照组微导管体外反复塑形的次数为(1.75±0.64)次,微导管末次塑形后从指引导管至到达动脉瘤腔的时间为82.50(46.25,108.75)s,首次塑形微导管后微导管抵达动脉瘤腔的成功率为35%(7/20),术中微导管塑形综合评分为(2.25±1.29)分,两组比较差异有统计学意义(P<0.05)。试验组患者术后2个月mRs评分为(0.60±0.68)分,对照组患者术后2个月mRs评分为(0.90±0.85)分,差异无统计学意义(P>0.05)。结论 相较于传统微导管塑形方式,3D打印技术辅助微导管塑形可减少术中微导管反复塑形次数、缩短操作时间、提高微导管抵达动脉瘤腔的成功率和微导管塑形的精确度。
