【摘要】 目的 探讨CT引导下经皮穿刺氩氦刀冷冻消融治疗平滑肌肉瘤的安全性及临床疗效。方法 选取2012年1月至2016年1月就诊的平滑肌肉瘤患者25例,采用CT引导下氩氦刀冷冻消融治疗,对治疗后局部靶病灶变化、术后疾病无进展生存(PFS)、生存率等进行动态随访观察,并记录并发症情况。结果 本组25例患者,术后即刻CT扫描根治冷冻融13例(52%),显著减瘤冷冻11例(44%),有效减瘤冷冻1例(4%)。术后3个月复查CT,按实体瘤评价标准,完全缓解(CR)14例,部分缓解(PR)8例,病灶稳定(5D)1例,缓解率(CR+PR)为88%。局部PFS(9.4±6.2)个月,1年生存率64%,2年生存率48%,3年生存率32%。本组病例未见出血、肿瘤溶解综合征等严重并发症,无手术相关死亡病例发生,术后出现发热、皮肤冻伤、术后即刻局部疼痛加重、神经损伤,对症处理后均消失或缓解。结论 CT引导下经皮穿刺氩氮刀冷冻消融术治疗平滑肌肉瘤近期疗效肯定,是一种安全可靠、值得推广的微创治疗方法。
