【摘要】 目的 探讨采用AngioJet血栓清除系统治疗急性肢体缺血(ALI)的有效性和安全性。方法 回顾性分析采用AngioJet血栓清除系统治疗19例下肢ALI患者临床资料,其中男14例,女5例;年龄66~90岁,平均(77.7±6.8)岁。根据血栓清除率>90%为Ⅲ级、50%~90%为Ⅱ级、<50%为Ⅰ级标准评价术后血栓清除率,记录术后相关并发症、截肢及死亡率。结果 19例患者手术技术成功率为100%。初次治疗后血栓清除率均>50%,其中Ⅲ级14例(73.7%),Ⅱ级5例(26.3%),下肢疼痛、缺血等症状均得到改善。术后5例出现一次性肉眼血红蛋白尿,无穿刺点出血、消化道出血、肾功能损伤及死亡等严重并发症发生。术后保肢成功17例,保肢率为89.5%;术后6个月保肢率为84.6%(11/13),1年为80.0%(8/10)。结论AngioJet血栓清除系统治疗下肢ALI安全有效,可快速恢复动脉血流,尤其适用于高龄、外科手术风险较大或有溶栓禁忌证患者。
