 【摘要】 目的 探讨CT和DSA复合引导下弹簧圈定位技术在肺小结节(small pulmonary nodule, SPN)胸腔镜切除术(video?蛳 assisted thoracoscopic surgery,VATS)中的临床应用。方法 采用高分辨CT发现的13例SPN患者,用飞利浦复合手术室,术前行CT扫描定位,穿刺置入弹簧圈,术中在DSA下行胸腔镜SPN楔形切除术。评价成功率、并发症、手术耗时及住院时间等。结果 共13例患者14处SPN行VATS,手术皆获成功,2例(15.4%)出现并发症,手术耗时0.5 ~ 2 h,平均(1.4 ± 0.6)h,住院时间8 ~ 14 d,平均(11.8 ± 3.8)d。SPN术后病理诊断原发性肺癌9例(69.2%),转移瘤1例(7.7%),错构瘤1例(7.7%),不典型瘤样增生2例(15.4%)。结论 本组不具有典型征象的SPN约76.9%组织学诊断是恶性病变, CT和DSA复合引导下弹簧圈标记定位胸腔镜SPN切除是一种介入微创手术,能高效、准确切除病灶并有效缩短手术及住院时间,减少并发症,提高SPN的诊断率,同时根治切除。
