摘要:
目的 探讨经导管植入蘑菇球型二尖瓣支架治疗二尖瓣反流的可行性和有效性。方法自行设计镍钛记忆合金材料的蘑菇球型支架。将猪心包经脱细胞处理后于0.6%戊二醛浸泡36 h,生理盐水洗净,裁剪成人工瓣膜,缝合在蘑菇球型支架底部,制成带瓣膜二尖瓣球形支架。手术前将蘑菇球型支架压缩至预载输送鞘管上。选择健康中华田园犬3只,小切口侧开胸暴露心尖,穿刺左心室心尖,送入加硬导丝至左心房至肺静脉。透视下沿加硬导丝将预载有蘑菇球型支架的20 F输送鞘管的头端送至左心房中部,缓慢后撤外鞘,逐步释放支架至左心房,蘑菇球型支架底部圆柱体正对二尖瓣开口。手术后即刻观察实验效果。结果 3只中华田园犬经导管带瓣膜蘑菇球型支架植入均获得成功,术后即时影像及解剖结果表明球形支架植入位置理想,二尖瓣反流明显减少。结论 自制蘑菇球型二尖瓣支架经导管植入治疗严重二尖瓣反流方法可行,效果理想。
