摘要:
目的 评估急性大血管闭塞型脑卒中患者机械取栓术后肺炎的风险因素并构建风险预测模型。方法 纳入昆明市延安医院和昆明市第一人民医院接受急性缺血性脑卒中取栓治疗的患者388例,2019年1月至2023年12月入院患者315例作为建模组,2024年1月至2024年9月入院患者73例作为外部验证组。收集人口统计学以及入院24 h的临床诊疗数据,分析机械取栓术后肺部感染的风险因素。采用7种机器学习算法构建大血管闭塞型脑卒中患者机械取栓术后肺部感染预测模型,以准确性、召回率、精确率、F1指数和受试者工作特征曲线下面积评价模型性能。结果 梯度提升分类器在建模组和验证组中准确率、召回率、精确率、F1指数和AUC值综合表现都较优。结论 基于机器学习有利于早期识别脑卒中机械取栓术后患者肺部感染及挖掘风险因素,及时采取防控措施,降低肺部感染发生率。
