 【摘要】 目的 利用CT灌注成像技术评价兔VX2后腿肌肉间移植瘤经抗血管生成药物不同给药方式治疗后肿瘤解剖形态及血管功能的改变,探讨不同给药途径的肿瘤抗血管生成治疗的疗效差异。方法 新西兰大白兔18只,建立兔后腿移植瘤动物模型,按数字法随机分成3组,每组6只,分别经静脉给药途径(静脉组)、局部瘤内注药途径(局部组)给予重组人血管内皮抑制素注射液和局部生理盐水注射(对照组)处理。治疗7 d后测量各组VX2肿瘤体积,利用CT灌注成像参数血流量(BF)、血容量(BV)和表面渗透性(PS)进行血管功能评价;与病理结果和免疫组化检测的微血管密度(MVD)进行对照分析。结果 建模成功率100%。静脉组及局部组肿瘤体积较对照组小(P < 0.05),静脉组及局部组抑瘤率分别为50.52%和56.21%。此外,局部组中有1只动物肿瘤出现明显坏死。CT灌注检查显示静脉组及局部组治疗后CT灌注参数BF、BV及PS较治疗前减少(P < 0.05),而对照组治疗前后灌注参数差异无统计学意义(P > 0.05)。另外,治疗后各组间CT灌注参数比较,静脉组与局部组间的BF、BV、PS差异无统计学意义(P > 0.05)。静脉组及局部组BF、BV、PS平均值均少于对照组(P < 0.05)。病理组织学检查显示静脉组及局部组切片中组织内可见大片坏死结构,对照组肿瘤内少量坏死。免疫组化检查显示静脉组与局部组间MVD差异无统计学意义(P > 0.05)。而静脉组与局部组MVD均低于对照组(P < 0.05)。相关性分析提示CT灌注参数与MVD之间均具有相关性。结论 CT灌注成像技术是评估肿瘤血管生成及血管功能状态的有效、便捷方法。抗血管生成药物的局部瘤内注射术是有效的抗肿瘤治疗方案。
