 【摘要】 目的 分析影响颅内多发性动脉瘤破裂的危险因素。方法 2007年7月—2011年7月收治134例多发性颅内动脉瘤患者，共294枚动脉瘤，其中50枚破裂，244枚未破裂。每例患者有2枚或2枚以上动脉瘤。采用单因素和多因素Logistic回归分析患者年龄、性别、动脉瘤大小、位置等因素。结果134例患者中，45 ～ 65岁患者动脉瘤破裂率最高。35例前交通动脉瘤中，13例破裂（37.1%），占所有破裂动脉瘤的26%（13/50）。前交通动脉瘤破裂率明显高于颅内其他位置的动脉瘤（P = 0.001）。294枚动脉瘤中，< 5 mm者占88.1%，< 3 mm者占58.2%。在破裂动脉瘤中，< 5 mm的动脉瘤占68%。结论 多发性颅内动脉瘤的破裂与患者年龄、动脉瘤位置和大小有关，应作为决定未破裂动脉瘤是否进行手术干预的条件。在多发性动脉瘤中，前交通动脉瘤最容易破裂。
