摘要:
目的探讨多层螺旋CT在判断肝癌寄生供血中的应用价值，分析寄生供血形成的原因。方法回顾性分析４０例经DSA证实存在寄生供血的肝癌患者的多层螺旋CT血管造影（MSCTA）表现，比较分析不同MSCTA重建方式在显示肝癌寄生供血方面与DSA的符合率。分析存在肝癌寄生供血病例的MSCTA特征。结果DSA显示寄生动脉５０支，MSCTA显示寄生动脉４０支，阳性率８０％。最大密度投影（MIP）和容积再现技术（VRT）两种重建方式结合能达到最佳显示效果。存在寄生动脉供血的病灶均位于肝脏边缘，长径平均为６．９cm。结论MSCTA在显示肝癌寄生供血方面与DSA有较高的符合率，可应用于介入治疗、外科手术以及肝移植术前的评价。肝癌病灶解剖部位临近裸区和悬韧带，以及病灶巨大对临近器官的直接侵犯、粘连可能是寄生动脉供血形成的主要原因；反复的TACE治疗和随之发生的肝动脉侧支供血减少也是寄生动脉供血形成的重要原因。
