【摘要】 目的 分析Aegisy和OptEase滤器未取出的危险因素,构建预测模型,指导临床更好地使用此类滤器。方法 收集2016年1月至2022年1月在常州第二人民医院接受Aegisy和OptEase滤器置入治疗患者的临床和影像资料。采用 logistic 回归筛选出未取出滤器的独立危险因素。根据回归分析结果构建风险列线图预测模型,并对模型的预测性和准确性进行内部验证。结果 共纳入826例患者,其中取出组522例,未取出组304例,取出率为63.2%。多因素logistic回归分析显示,年龄、深静脉血栓形成(DVT)是否行介入治疗、骨折、活动性恶性肿瘤、长期卧床、住院期间ICU入住史、抗凝禁忌证、下腔静脉(IVC)血栓形成、肺栓塞(PE)既往史、滤器置入-出院间隔是否≥14 d等是滤器未取出的危险因素。将多因素 logistic回归结果中的危险因素作为预测因子构建列线图预测模型,检验结果表明模型准确性和区分度良好,一致性指数为0.861(95%CI=0.776~0.905)。结论 基于年龄、DVT是否行介入治疗、骨折、活动性恶性肿瘤、长期卧床、住院期间ICU入住史、抗凝禁忌证、IVC血栓形成、PE既往史、滤器置入-出院间隔是否≥14 d建立的预测Aegisy和OptEase滤器未取出风险的列线图模型,具有良好的预测效能和较高的临床价值。
