【摘要】 目的 探讨下腔静脉滤器(IVCF)断裂的相关并发症及治疗策略。方法 检索PubMed数据库中有关IVCF断裂的文献,对相关数据进行汇总分析。结果 共纳入59篇研究文献(个案31篇,论著28篇),涉及16种类型IVCF(永久型5种,可回收型11种),共4 392例患者。其中223例患者发生IVCF断裂,中位发生时间为滤器置入后30.4个月。断裂的支撑杆位置依次为下腔静脉(54.0%)、肺动脉(19.9%)、右心室(14.2%)、其他(11.9%)。80例(35.9%)有断裂支撑杆处理措施,如随访观察(65.0%)、腔内介入取出(17.5%)、外科手术取出(15.0%)、药物保守治疗(2.5%);88例(39.5%)有断裂IVCF处理措施,如随访观察(61.4%)、腔内介入取出(35.2%)、外科手术取出(3.4%)。IVCF断裂相关严重并发症8例(3.6%),其中心脏压塞6例,完全性房室传导阻滞1例,猝死1例。 结论 各种IVCF均可发生断裂。断裂的支撑杆近半数分布于远隔脏器,1/5患者有临床症状,15%需外科干预,3.6%出现严重并发症,甚至死亡。
