【摘要】 目的 了解肺癌125I粒子植入术后SPECT/CT放射性浓聚计数值与剂量分布及疗效的关系。方法 选取2019年1月至2021年5月行125I粒子植入治疗的肺癌患者11例。处方剂量(prescription dose,PD)140 Gy,粒子活度0.5~0.8 mCi,粒子数20~77颗。125I粒子植入术后首日行SPECT/CT扫描,将SPECT图像与CT图像融合,调节融合图像的放射性浓聚感兴趣区域(region of interest,ROI),找到放射性计数值为200及300所对应的曲线,截取包含肿瘤各层面完整图像,将图像传入计算机治疗计划系统(treatment planning system,TPS),勾画肿瘤靶区及危及器官。计算计数值200及300曲线所覆盖的体积与肿瘤靶体积之比RV200及RV300。随访2~4个月,复查胸部CT并导入TPS,计算肿瘤体积缩小值与原肿瘤靶体积之比RVt及平均每月体积缩小比R1。分别将RV200、RV300与R1关联并得出关系曲线及公式。结果 VC200、VC300、V、RV200、RV300、R1分别为(47.02±29.85) cc、(29.18±21.85) cc、(42.27±31.99) cc、(1.22±0.29)、(0.75±0.28)、(0.23±0.13)。计数值分别为200、300体积覆盖比与肿瘤体积每月缩小比呈对数关系,拟合公式为:R1=0.53×log(RV200)+ 0.13;R1=0.50×log(RV300)+0.36。 结论 SPECT/CT放射性浓聚计数值为基础的参数与125I粒子术后靶体积变化有相关性,当RV200>1.09时疗效更好,可预测肺癌125I粒子植入后的疗效。
