摘要:
目的改良和评价建立犬颈动脉侧壁动脉瘤模型的方法。方法以显微外科缝合技术在１５条犬的双侧颈动脉建立多枚动脉瘤模型。随机分为３组，分别采用术中肝素化（A组）、术后肝素化（B组）；不肝素化（C组）３种处理，于模型建立后２周及１个月行血管造影确认结果。２周、１个月时随机取材行动脉瘤病理学检查，对不同抗凝处理条件动物２周时的模型建立结果进行统计学比较。结果成功建立了３７枚侧壁宽颈动脉瘤，血管造影示血流从动脉瘤远侧壁进入瘤内，沿着瘤壁从近侧壁流出，瘤腔中心有涡流。C组有６枚、A组有１枚动脉瘤自闭。３组共有５枚动脉瘤内有部分血栓形成，A组与B组间血栓形成率无差异（Nemenyi法：P＞０．５）。组织病理学提示２周时的动脉瘤内有较明显的内膜增生和炎症反应，瘤颈处缝线无完整内膜覆盖。１个月时动脉瘤瘤壁炎症反应消退，瘤颈处缝线有完整内膜覆盖，流入道受血流冲击处可见瘤壁内膜增生。结论建模时术后肝素化与术中肝素化可达到同样的抗凝效果，外科缝合建立多枚动脉瘤可控性好，并可实现自身对照和空白对照，在对动脉瘤治疗技术的研究中有一定的优势。
