摘要:
目的探讨经皮椎体成形术(PVP)中同轴活检的安全性、操作技术及病理诊断意义。方法64例疑为病理性压缩骨折共85节椎体在PVP术中注入骨水泥前先行同轴活检。采用C臂透视监视下经椎弓根入路行椎体穿刺,当骨穿针头端抵达椎体后1/4处时置入活检针多方向、多次切取组织,取出组织后再将骨穿针推进至椎体前1/3处注入骨水泥。术后1d行CT复查观察骨水泥分布及有无渗漏,随访1个月观察有无并发症。结果85节椎体中4节(4.7%)取出的组织量不足以行组织学诊断,明确病理诊断58例(90.6%)。有脊椎外原发肿瘤史38例,病理诊断椎体转移癌29例(76.3%),骨质疏松7例(18.4%),诊断不明2例(5.3%)。无脊椎外原发肿瘤史26例,病理诊断转移癌10例(38.5%),骨髓瘤9例(34.6%),骨质疏松4例(15.4%),诊断不明3例(11.5%)。无严重并发症发生。结论PVP术中同轴活检操作方法简单、安全、可行,病理检出率高,增加组织取出量的关键是从椎体破坏明显侧进针,多方向、多次切取。
