摘要:
目的 观察抗凝联合血管内治疗(endovascular therapy,EVT)重型脑静脉窦血栓形成(cerebral venous sinus thrombosis,CVST)的有效性和安全性。方法 回顾性分析2018年至2023年在郑州大学第一附属医院接受抗凝联合EVT治疗的40例重型CVST患者临床资料。比较入院和出院时改良Rankin量表(mRS)、美国国立卫生研究院卒中量表(NIHSS)、Glasgow昏迷量表(GCS)评分,以及血管再通率、随访预后、临床症状改善、并发症情况。结果 40例重症CVST患者中男12例(30.0%)、女28例(70.0%); 接受联合疗法31例(77.5%)、单纯置管溶栓4例(10.0%)、支架取栓3例(7.5%)、导管抽栓1例(2.5%)、球囊扩张1例(2.5%)。入院和出院时中位mRS评分由4(4,5)下降为0(0,1),NIHSS评分由8(6,12)下降为0(0,1),GCS评分由11(10,11)上升为15(15,15),差异有统计学意义(均P<0.001)。出院前复查磁共振静脉造影(MRV)或DSA显示,静脉窦完全再通17例(42.5%)、部分再通22例(55.0%),1例(2.5%)未再通。出院时31例(77.5%)患者预后良好(mRS评分≤2); 随访6个月时33例(82.5%),12个月时34例(85.0%)患者预后良好。随访期末症状痊愈或好转率,头痛为92.0%(23/25),恶心呕吐为100%(15/15),视觉障碍为83.3%(5/6),运动或感觉障碍为87.5%(14/16),意识障碍为90.0%(18/20),癫痫为94.4%(17/18),昏迷为83.3%(5/6)。随访结束时有5例(12.5%)患者出现并发症,其中3例(7.5%)死亡。结论 抗凝联合EVT治疗重型CVST有效且安全。
