摘要:
目的 探讨大分割放疗(HFRT)联合经导管动脉化疗栓塞术(TACE)治疗鼻咽癌异时性肝寡转移的效果和安全性。方法 回顾性收集2012年1月至2022年10月四川省肿瘤医院采用HFRT联合TACE治疗的鼻咽癌异时性肝寡转移患者临床资料。分析治疗后患者3年和5年总生存期(OS)、无进展生存期(PFS)、局部无复发生存期(LRFS),无肝外远处转移生存期(EMFS)及治疗相关不良反应。结果 共纳入55例患者,其中男36例,女19例,发生异时性肝寡转移时中位年龄为44(27～75)岁。所有患者治疗后3、5年OS比率分别为67.8%、40.0%,PFS比率分别为55.8%、30.0%,LRFS比率分别为72.7%、56.5%,EMFS 比率分别为63.6%、56.5%。亚组分析显示,≥3周期TACE治疗可显著改善患者PFS。所有患者总体治疗耐受性良好,3～4级不良反应发生率低。结论 HFRT联合TACE治疗鼻咽癌异时性肝寡转移效果明确,患者耐受性良好。
