【摘要】 目的 分析结直肠癌肝转移患者外周血单个核细胞中含表皮生长因子的纤维蛋白样细胞外基质2（EGF containing fibulin-like extracellular matrix protein 2，EFEMP2）水平与TACE治疗疗效的关系。 方法 选择2015年3月至2017年7月在海安市中医院接受治疗的152例结直肠癌肝转移患者作为研究对象。用Ficoll密度梯度分离法分离外周血中单个核细胞；用蛋白质印迹法检测单个核细胞中EFEMP2表达水平。分析EFEMP2表达水平与TACE治疗疗效的关系。结果 生存组和死亡组EFEMP2水平均随时间进展呈升高趋势。生存组不同时间节点时EFEMP2水平均高于死亡组，差异均有统计学意义（P＜0.05）。高EFEMP2组中位生存时间高于低EFEMP2组，差异有统计学意义（P＜0.05）。Cox回归分析结果显示靶向治疗和T3-EFEMP2均是结直肠癌肝转移患者3年生存预后的独立保护因素（P＜0.05）。限制性立方样条拟合Cox回归分析结果显示T3-EFEMP2与结直肠癌肝转移患者3年生存预后呈非线性关系（P＜0.05）。结论 直肠癌肝转移患者外周血单个核细胞中EFEMP2水平与TACE治疗疗效有关，其水平低提示TACE治疗临床获益较小。
