【摘要】 目的 探讨采用AngioJet血栓清除系统治疗急性肢体缺血（ALI）的有效性和安全性。方法 回顾性分析采用AngioJet血栓清除系统治疗19例下肢ALI患者临床资料，其中男14例，女5例；年龄66～90岁，平均（77.7±6.8）岁。根据血栓清除率＞90%为Ⅲ级、50%～90%为Ⅱ级、＜50%为Ⅰ级标准评价术后血栓清除率，记录术后相关并发症、截肢及死亡率。结果 19例患者手术技术成功率为100%。初次治疗后血栓清除率均＞50%，其中Ⅲ级14例（73.7%），Ⅱ级5例（26.3%），下肢疼痛、缺血等症状均得到改善。术后5例出现一次性肉眼血红蛋白尿，无穿刺点出血、消化道出血、肾功能损伤及死亡等严重并发症发生。术后保肢成功17例，保肢率为89.5%；术后6个月保肢率为84.6%（11/13），1年为80.0%（8/10）。结论AngioJet血栓清除系统治疗下肢ALI安全有效，可快速恢复动脉血流，尤其适用于高龄、外科手术风险较大或有溶栓禁忌证患者。
