 【摘要】 目的 探讨颈内动脉眼动脉段动脉瘤血管内治疗的安全性和有效性。方法 2008年7月—2013年8月收入眼动脉段动脉瘤患者23例，行血管内栓塞治疗。按临床Hunt?蛳 Hess 分级，Ⅰ级11例，Ⅱ级10例，Ⅲ级2例。动脉瘤直径为2.2 ～ 19.9 mm，平均5.7 mm。术后采用DSA或MRA及改良Rankin量表评分进行影像和临床随访。结果 23例患者共有27枚动脉瘤，其中采用单纯GDC弹簧圈栓塞15枚，Solitaire AB支架辅助栓塞12枚。动脉瘤即刻栓塞结果按Raymond分级Ⅰ级22枚，Ⅱ级4枚，Ⅲ级1枚。手术相关并发症3例，1例术中瘤颈出现血栓形成，1例支架辅助栓塞术中1枚弹簧圈尾端突入支架内，1例栓塞术后出现一过性失明。18例患者随访2 ~ 62个月，复查造影显示动脉瘤完全栓塞15例（84%），次全栓塞3例，1例患者存在双侧动脉瘤，未处理侧随访体积增大，并给予血管内治疗。临床随访中，改良Rankin评分0 ~ 1分18例，均病情稳定，无新发的神经功能障碍及出血。结论 血管内栓塞治疗眼动脉段动脉瘤具有微创、安全、有效的优点，远期疗效和安全性仍需进一步研究。
