摘要:
目的探讨经皮椎体成形术（PVP）中同轴活检的安全性、操作技术及病理诊断意义。方法６４例疑为病理性压缩骨折共８５节椎体在PVP术中注入骨水泥前先行同轴活检。采用C臂透视监视下经椎弓根入路行椎体穿刺，当骨穿针头端抵达椎体后１／４处时置入活检针多方向、多次切取组织，取出组织后再将骨穿针推进至椎体前１／３处注入骨水泥。术后１d行CT复查观察骨水泥分布及有无渗漏，随访１个月观察有无并发症。结果８５节椎体中４节（４．７％）取出的组织量不足以行组织学诊断，明确病理诊断５８例（９０．６％）。有脊椎外原发肿瘤史３８例，病理诊断椎体转移癌２９例（７６．３％），骨质疏松７例（１８．４％），诊断不明２例（５．３％）。无脊椎外原发肿瘤史２６例，病理诊断转移癌１０例（３８．５％），骨髓瘤９例（３４．６％），骨质疏松４例（１５．４％），诊断不明３例（１１．５％）。无严重并发症发生。结论PVP术中同轴活检操作方法简单、安全、可行，病理检出率高，增加组织取出量的关键是从椎体破坏明显侧进针，多方向、多次切取。
