【摘要】 目的 探讨1.5 T MRI引导下经皮射频消融(RFA)治疗结直肠癌肝转移的疗效。方法 回顾性分析2011年5月至2017年12月接受1.5 T MRI引导下RFA治疗结直肠癌肝转移患者42例结肠癌23例,直肠癌19例,肝内共83个转移灶,病灶平均直径(1.70±0.76) cm(0.65~3.64 cm),在MR引导下完成穿刺点定位、射频电极穿刺及消融位点调整,观察RFA术后即刻MRI表现及并发症,随访复查判定消融疗效。结果 所有患者均顺利完成1.5 T MRI引导下RFA治疗。术后即刻行MRI扫描,T1WI上消融灶呈特征性“靶征”,T2WI上消融灶中央呈低信号,周边见薄环状高信号影环绕。1例患者术后出现严重并发症(2.38%,1/42)。术后平均随访时间(41.7±20.7)个月(15~94个月),79个肝转移灶完全消融(95.18%,79/83),4个转移灶出现局部肿瘤进展(4.82%,4/83)。26例患者出现肝内新发病灶,中位进展时间为5.5个月(1~65个月)。RFA后1、3、5年的总生存率分别为73.8%、40.4%和15.2%。结论1.5 T MRI引导下经皮射频消融治疗结直肠癌肝转移是一种安全、有效的方法。
