【摘要】 目的 采用CT导引下经皮穿刺“体外预装示踪一步植入技术”制作兔VX2肝/肾肿瘤模型,评价该方法的有效性及便捷性。方法 采用一次性16 G胸穿刺针(8.5 cm)和自制18 G穿刺探针进行接种。胸穿刺针头腔内逆行装填1.5 mm×1.5 mm×3.0 mm明胶海绵条并用0.3 mL对比剂浸润,再逆行装入1.0 mm×1.0 mm×3.0 mm VX2组织块1条完成体外预装。CT导引下预装穿刺针经皮穿刺实验兔靶器官,确认针尖到达理想靶点(种植肝肿瘤于肝左叶外侧段,肾肿瘤于肾下极),将针尾端插入探针并推出内装的肿瘤组织块和明胶海绵,30 s后缓慢地整体拔出穿刺针和探针并按压穿刺点60 s,CT扫描明确高密度明胶海绵位置。记录植瘤时间。2、3、4周后增强CT确认建模结果。结果 术后即刻CT显示植瘤部位表现为肝内/肾内结节状高密度影。肝肿瘤造模10只兔,均获成瘤(10/10),平均植瘤时间4.3 min;肾肿瘤造模10只兔,成瘤9只(9/10),平均植瘤时间4.9 min。所有成瘤均为单发,成瘤部位与高密度明胶海绵部位基本一致。随访观察显示肝种植瘤CT增强为周边强化,DSA为环状强化,肾种植瘤CT增强及DSA均表现为肾实质染色缺损。所有接种肿瘤3~4周后快速增长明显。结论 CT导引下“体外预装示踪一步植入技术”制作兔VX2肝/肾肿瘤模型,具有便捷省时、术后即刻确认接种部位、成瘤率高且孤立成瘤等优点。
