【摘要】 目的 研究不同直径125I粒子放射性食管支架在肿瘤靶区的剂量学分布。方法 用激光扫描仪分别扫描画有直径12、14和16 mm的圆形并标记有5 cm刻度的白纸,以JPEG格式存储在计算机桌面上,根据圆形直径模拟相应食管支架并依次分为A、B、C 3组,利用图像转换程序,每组分别创建17层图片,每层图片层距为5 mm,将图片传输到计算机治疗计划系统(TPS)中,模拟不同直径,长度8 cm食管支架,利用TPS勾画肿瘤靶区(gross tumour volume,GTV),以模拟食管支架边界为肿瘤靶区内界,外扩0.5 cm为外界,于第5层面开始勾画环形并且长度为4 cm的靶区,设定相同处方剂量,并于该层面至第13层面于模拟食管支架上先后7次载入0.3、0.4、0.5、0.6、0.7、0.8和0.9 mCi 125I粒子,粒子层间距1 cm,每层平均布源4颗,得出剂量体积直方图(dose volume histogram DVH),记录每组D90、V90。结果 A、B、C 3组的D90分别为(77.24±19.92)、(69.56±25.27)和(56.38±20.08)Gy(F=0.84,P=0.44),差异无统计学意义;A、B、C 3组的V90分别为(77.76±30.73)%、(76.79±25.92)%和(64.10±32.49)%,(F=0.46,P=0.64),差异无统计学意义。结论 放射性食管支架直径为12 mm时,推荐粒子活度为0.6 mCi;直径为14、16 mm时,推荐粒子活度为0.7 mCi,但当直径16 mm,推荐每层布源5颗,该3种直径的食管支架对剂量学参数无明显影响。
