摘要:
自20世纪60年代以来,随着介入放射学技术的快速发展,液体栓塞剂已成为治疗肿瘤、出血及血管畸形等疾病的重要栓塞材料。该文系统介绍了常用液体栓塞剂的类型,包括碘化油、硬化剂(如无水乙醇和聚桂醇)、氰基丙烯酸酯类(如NBCA和Glubran胶)以及乙烯乙烯醇类(如Onyx),分析各自的栓塞机制、临床应用、优势及操作限制。同时介绍了近年来出现的新型液体栓塞剂,如新型聚合剂(如Magic GlueTM)、沉淀剂(如Squid和PHIL)、相变剂(如GPX)和温度敏感型水凝胶(如肽普绅TM)等,这些新型材料在生物相容性、操作灵活性和临床疗效方面均取得显著进步。此外,该文还讨论了液体栓塞剂在急慢性出血控制、肿瘤的综合介入治疗、复杂血管畸形和内脏动脉瘤治疗中的应用前景,并指出未来的研究方向将集中在新材料开发、多模态治疗方法结合、药物载体融合、人工智能和影像引导技术的应用以及个性化栓塞剂设计,以进一步提升液体栓塞剂的临床疗效和安全性。
