【摘要】 目的 探讨CT引导下经皮穿刺活检术诊断肝细胞癌(HCC)微血管侵犯(MVI)的临床价值。方法 回顾性分析2013年1月至2019年12月徐州医科大学影像学院行CT引导下经皮肝穿刺活检患者的临床资料及病理样本。将纳入的107例HCC患者作为实验组,文献中行外科手术切除的339例HCC患者作为对照组,对筛选的MVI相关指标进行对比分析,检测两组的一致性;以对照组MVI阳性率作为参考标准,预测实验组MVI阳性率,并将实验组穿刺标本诊断的MVI阳性率与预测的MVI阳性率和文献报道的术后标本检测的MVI阳性率进行对比分析。结果 两组除肿瘤直径和肿瘤数目以外的MVI相关指标间差异无统计学意义(均P>0.05),具有部分一致性;以对照组为参考标准,预测实验组MVI阳性率≥46.3%;实验组穿刺标本诊断的MVI阳性率为5.6%(6/107),明显小于最低预测值(46.3%)及术后标本检测值(15.0%~57.1%)。结论 CT引导下经皮穿刺活检术诊断HCC MVI的阳性率较低,无法准确反映HCC MVI的真实状态,临床应用具有局限性。
