 【摘要】 目的 评价肝硬化合并肝癌TACE治疗患者的肝功能储备，并以TACE治疗前后的临床指标对照，寻求简单无创的检查方法评价肝硬化患者的肝脏储备功能。方法 51例肝硬化合并肝癌TACE手术患者，以TACE术前及术后4周的Child?蛳 Pugh分级为标准，分为功能恶化组及稳定组。MR?蛳 DWI为研究手段，患者均于术前及术后4周行MR检查，分别测量不同肝段组织的表现弥散系数（ADC）值。比较恶化组与稳定组的临床资料及ADC值，同时比较治疗前后两组ADC值的变化。结果 恶化组与稳定组在TACE治疗前相关临床指标及Child?蛳 Pugh分级无明显统计学差异。恶化组的ADC值在治疗前后分别为（1.43 ± 0.20） × 10-3，（1.17 ± 0.18） × 10-3 mm2/s；稳定组的ADC值为（1.76 ± 0.17） × 10-3，（1.58 ± 0.23） × 10-3 mm2/s，两组在治疗前差异有统计学意义。恶化组治疗前后差异有统计学意义，稳定组治疗前后差异无统计学意义。结论 ADC值可以评估肝硬化合并肝癌患者的肝脏储备功能；随着ADC值下降，其储备功能受损，表现为TACE后肝功能恶化。
