摘要:
目的回顾性分析合并非梗死相关动脉（non－IRA）慢性完全阻塞的急性ST段抬高心肌梗死（STEMI ）患者的临床特点和急症经皮冠状动脉介入治疗（PCI ）疗效。方法３６０例STEMI 接受直接PCI 患者中，４７例至少１支non－IRA慢性阻塞（甲组），另３１３例无non－IRA阻塞（乙组）。比较两组的临床资料、介入治疗及随访结果。结果与乙组相比，甲组伴糖尿病患者比率增多（２５．５％比１４．１％，P＝０．０４３），既往心肌梗死史较多（１０．６％比３．２％，P＝０．０１７），PCI 时较多应用主动脉球囊反搏（IABP）（２１．３％比１０．９％，P＝０．０４２）、临时起搏（３８．３％比２１．１％，P＝０．００９）和心脏电复律（２１．３％比６．４％，P＜０．０１）。随访期中（平均４２个月），甲组因心力衰竭再次入院率增高（２１．３％比１０．９％，P＝０．０４２），但两组住院期（６．４％比５．４％）和随访期病死率（４．３％比５．８％）无明显差异。结论尽管合并non－IRA慢性完全阻塞的STEMI 患者病情凶险，但辅助循环和心电支持下直接PCI 仍可改善其近期和远期预后。
