【摘要】 目的 探讨碳花青荧光染料(CM- DiI)标记的自体骨髓内皮祖细胞(EPC)在兔终板下椎体缺血模型归巢和治疗效果。方法 分次抽取48只模型兔骨髓,分离出EPC并以CM- DiI标记计数;随机分成两组,A组(n=24)经腰动脉途径自体移植CM- DiI标记EPC,B组(n=24)经耳缘静脉途径自体移植,每兔移植EPC 4×106个。C组(n=18)模型兔未抽髓,为空白对照。移植术后7 d、1个月荧光显微镜下观察A、B组EPC归巢与血管生成,术后1、2、3个月对兔缺血椎体作MR检查,分析3组椎体缺血区血管计数变化。结果 A组移植术后7 d 红色荧光标记的EPC在终板下椎体缺血区归巢,术后1个月局部血管形成,术后1、2、3个月MRI显示缺血区小血管数逐渐增多,缺血区信号逐渐恢复正常,相应椎间盘未见退变表现;B、C组未见EPC归巢,血管计数逐渐减少。单因数方差分析(ANOVA)显示A组术后1、2、3个月血管计数稳步增加,缺血区血管生成、血管数逐渐增多,均优于B组、C组(P<0.05);最小显著性差异(LSD)法两两多重比较显示,A组每一时点血管计数均优于B组、C组(P<0.05)。结论 经腰动脉自体移植EPC可在终板下椎体缺血区归巢并使血管形成,预防缺血所致椎间盘退变相关疾病。
