【摘要】 目的 探讨CT引导下经皮胃造瘘术治疗肌萎缩侧索硬化所致吞咽困难的安全性。方法回顾性分析24例肌萎缩侧索硬化患者，有不同程度的呼吸困难，使用简易无创呼吸机辅助呼吸，伴吞咽困难，不能正常进食，行CT引导下经皮胃造瘘术，观察术中、术后呼吸功能及并发症。结果 所有患者均成功实施CT引导下经皮胃造瘘术，技术成功率为100%。术中及术后1、3 d无创血压（NIBP）、心率（HR）和血氧饱和度（SpO2）和术前差异无统计学意义，P＞0.05。随访3个月以上，1例患者出现胃造瘘口渗液，给予更换16 F胃造瘘管后渗液消失，1例患者出现胃造瘘口感染，经抗炎治疗后，感染痊愈。另1例患者2个月后出现胃造瘘管脱出，给予更换胃造瘘管。未发生严重并发症和治疗相关死亡。结论 CT引导下经皮胃造瘘术不影响肌萎缩侧索硬化患者的呼吸功能，而且相关并发症少，值得临床推广。
