【摘要】 目的 评估三维时间飞跃法(3D- TOF)MRA在筛选温岭社区人群颅内动脉瘤中的诊断价值。方法 2011年9月至2012年8月温岭社区2 124例颅内动脉瘤可疑患者纳入研究,均接受3D- TOF MRA和3D- DSA检查。分析温岭社区人群颅内动脉瘤流行病学数据,3D- TOF MRA检测颅内动脉瘤的有效性及其与3D- DSA检查金标准的一致性。结果 3D- DSA检查显示温岭社区人群颅内动脉瘤发生率为6.87%(146/2 124),其中男71例(48.63%),女75例(51.37%),平均年龄(41.23±11.56)岁;伴随疾病包括高血压、糖尿病、动脉硬化、脑血管疾病。3D- TOF MRA检查显示149例颅内动脉瘤,其中5例为误诊,2例为漏诊,检查灵敏度为98.63%(144/146),特异度为99.72%(1 773/1 778),准确度为99.67%(2 117/2 124)。两种方法检测颅内动脉瘤纵径及颈宽的差异均无统计学意义(P>0.05)。结论 3D- TOF MRA检查颅内动脉瘤具有较高的灵敏度、特异度和准确度,其无创性优势更适用于颅内动脉瘤初筛。
