 【摘要】 目的 通过动态增强对比磁共振成像(DCE?蛳 MRI)量化经皮血管腔内成形术(PTA)术后糖尿病患者下肢内皮细胞功能变化,进而探讨预测PTA术后下肢血管再狭窄的可能性。方法 收集2009年5月—2012年1月行膝下动脉PTA治疗的糖尿病下肢血管病变患者 51 例(77 条患肢),20名正常志愿者作为对照组,通过DCE?蛳 MRI扫描获得比目鱼肌的跨膜转运常数(K?蛳 trans)值。糖尿病组以6个月随访结果分为再狭窄与非狭窄,比较再狭窄与非狭窄患者的K?蛳 trans值,同时记录糖尿病组和对照组的一般临床资料。结果 尽管在术后再狭窄与非狭窄患者的临床体征得到改善,但再狭窄患者的K?蛳 trans值无明显改善,非狭窄患者K?蛳 trans值明显改善,而二者在术前K?蛳 trans值无明显差异,再狭窄患者与对照组比较差异有统计学意义(P < 0.05)。结论 糖尿病患者下肢K?蛳 trans值可以反映毛细血管内皮功能,可预测PTA术后再狭窄,为临床治疗提供依据。
