 【摘要】 目的 建立一种可模拟人椎体成形术中骨水泥椎间盘渗漏的动物模型。方法 采用成年健康杂种犬8只,每只犬取L2/3 ~ L5/6共4个椎间盘为实验对象,4个椎间盘随机分入A、B、C、D组,每组8个。A组和B组用18 G穿刺针穿刺椎间盘纤维环,A组不注入任何物质, B组注入0.3 ml聚甲基丙烯酸甲酯(PMMA);C组和D组用16 G穿刺针穿刺椎间盘纤维环,C组不注入任何物质,D组注入0.3 ml PMMA。分别于造模后6、12、24周行MR检查,计算MR指数并对其进行统计分析。结果 MR检查发现,A组髓核、纤维环结构完好,纤维环呈均匀低信号,T2加权像髓核呈均一高信号。C组T2加权像髓核信号不同程度降低且不均一,髓核中心可见不规则的斑点状低信号,其相对高信号区面积减小。B、D组椎间隙相应上下椎体面变毛糙。T2加权像髓核信号不同程度降低且不均一,髓核中心可见不规则的斑点状更低信号,髓核形态不规则,其相对高信号区面积不同程度减小。造模后6周, B组与D组的MR指数差异无统计学意义(P > 0.05),但12和24周时组间差异均有统计学意义(P < 0.05)。造模后6、12、24周,B、C、D组MR指数比较,组间差异均有统计学意义(P < 0.05)。结论 18 G穿刺针单次穿刺犬椎间盘纤维环不会导致其退变,用18 G穿刺针经皮穿刺犬椎间盘纤维环注射骨水泥可建立骨水泥椎间盘渗漏模型,具有创伤小、模型制作成功率高等优点。此动物模型适合于研究骨水泥椎间盘渗漏对椎间盘生理、生化及病理等影响的机制。
