摘要:
目的 测量不同投照角度下桡动脉入路经皮冠状动脉介入治疗(percutaneous coronary intervention,PCI)患者不同解剖位置辐射剂量分布情况,分析辐射防护设备研制的应用效果。方法 模拟桡动脉入路PCI常规操作,对模拟患者的标准体模进行透视采集,测量不同投照角度下体模不同解剖位置辐射剂量强度及衰减情况。绘制标准体模受照辐射强度剂量图。为标准体模辐射防护研制X线防护头罩和铅裙。采用t检验对比分析不同投照角度下标准体模晶状体、甲状腺和性腺有或无防护设施时辐射强度差异。结果 标准体模受照辐射强度剂量图显示,三维空间中不同投照角度下不同解剖位置剂量梯度变化。相同采集参数下,使用X线防护头罩和铅裙的标准体模晶状体(t=35.77~210.83,P<0.001)、甲状腺(t=26.69~118.15,P<0.001)和性腺(t=33.16~155.55,P<0.001)在不同投照角度下的辐射强度显著低于无防护措施时,差异有统计学意义。结论 患者标准体模不同解剖位置辐射强度与投照角度有关。使用研制的X线防护头罩和铅裙可显著降低桡动脉入路PCI患者关键解剖部位受照辐射剂量,有明显的辐射防护作用。
