摘要:
目的 采用实时辐射剂量计评估不同术式经动脉栓塞治疗术者的辐射剂量情况,并分析术者接受辐射剂量的影响因素。方法 前瞻性采集宁夏医科大学总医院2024 年4 月至12 月接受经动脉栓塞治疗的83 例患者和术者基本资料,以及术中辐射剂量相关指标。按手术类型分为5 组,肝动脉化疗栓塞术组(TACE组)39 例、支气管动脉栓塞术组(BAE组)14 例、肾动脉栓塞术组(RAE)13 例、子宫动脉栓塞术组(UAE组)11 例和膝动脉栓塞术组(GAE组)6 例。数据采用Kruskal-Wallis H和Mann-Whitney U检验,相关性分析采用Spearman相关性分析法,以P<0.05表示差异有统计学意义。结果 TACE组、BAE组、RAE组、UAE组和GAE组的透视时间(P<0.001)、透视DAP(P=0.002)、透视AK(P=0.022)、第一术者辐射剂量(P<0.001)和第一术者辐射剂量率(P<0.001)中位数差异均具有统计学意义,其中GAE组的透视时间、第一术者辐射剂量和第一术者辐射剂量率最高,UAE组的透视DAP和透视AK最大。TACE组、BAE组、RAE组、UAE组和GAE组第一术者和第二术者辐射剂量差异有统计学意义(均P<0.05),且第一术者接受的辐射剂量显著高于第二术者; TACE组、BAE组、RAE组和UAE组中不同第一术者身高(身高≤175 cm和身高>175 cm)组、不同FOV(正常和缩小)组和不同患者BMI(BMI<18.5 kg/m2、BMI 18.5~24.9 kg/m2和BMI≥25 kg/m2)组的第一术者辐射剂量率差异均有统计学意义(均P<0.05),且术者身材矮的比身材高的第一术者接受的辐射剂量高、第一术者接受的辐射剂量随着患者BMI的增大而增大、第一术者接受的辐射剂量随着FOV的缩小而减少。结论 不同术式经动脉栓塞治疗术者接受的辐射剂量及辐射剂量相关指标存在差异,可以通过减少透视时间、透视DAP、增加与辐射源的距离、增高术者的站位和减小FOV等降低术者接受的辐射剂量。
