摘要:
目的 分析不同基线桡动脉直径患者应用血流介导的血管扩张(flow-mediated dilation,FMD)后桡动脉直径的时序变化轨迹,并初步探讨其对桡动脉穿刺的潜在影响。方法 采用前瞻性队列研究设计,连续纳入2024年1~7月拟行脑血管造影的113例患者,依据术前超声测量的基线桡动脉直径分为3组:1组(2.5~3.0 mm,n=35)、2组(<2.5 mm,n=54)、3组(>3.0 mm,n=24)。应用FMD技术(肱动脉加压至收缩压+30 mmHg,持续5 min阻断血流),于加压释放后即刻(舒张末期)、5、10、15 min通过超声测量桡动脉直径,分析其变化轨迹,同时记录穿刺次数、穿刺是否成功和鞘管置入时间,比较组间差异。结果 1组加压释放后的桡动脉直径变化呈现轻度减小、2组直径维持不变、3组直径变化呈现显著减小,3组首次穿刺成功率(1组85.7%、2组83.3%、3组87.5%)、平均穿刺次数(均为1.1次)及鞘管置入时间(约5.1 min)差异均无统计学意义(P>0.05)。结论 基线桡动脉直径<2.5 mm的患者在FMD后5 min内桡动脉直径稳定性较好,而较大直径血管则较早出现回缩趋势,FMD可导致不同基线桡动脉直径产生差异性的扩张后变化轨迹。
