摘要:
目的 探讨DSA引导下微波消融(microwave ablation,MWA)结合药物洗脱微球-经导管动脉化疗栓塞术(transcatheter arterial chemoembolization with drug eluting beads,DEB-TACE)治疗原发性肝癌(primary liver cancer,PLC)患者的效果及预后。方法 纳入2022年1月至2023年12月济南市人民医院收治的121例PLC患者。根据随机数字表法将患者分为对照组(n=60,DEB-TACE治疗)和观察组(n=61,DSA引导下MWA结合DEB-TACE治疗)。比较两组患者疗效、肝功能、肿瘤标志物、预后及并发症,分析影响PLC患者总生存期的因素。结果 观察组客观缓解率、疾病控制率分别为85.24%(52/61)、90.16%(55/61),均高于对照组70.00%(42/60)、76.67%(46/60)(均P<0.05); 治疗后3个月观察组血清天冬氨酸转氨酶、总胆红素及丙氨酸转氨酶含量低于对照组(均P<0.05),血清癌胚抗原、甲胎蛋白、糖类抗原125含量低于对照组(均P<0.05),无进展生存期、总生存期高于对照组(均P<0.05); 随访1年观察组无进展生存率、总生存率分别为60.66%(37/61)、81.97%(50/61),高于对照组41.67%(25/60)、65.00%(39/60)(均P<0.05)。两组间并发症比较差异无统计学意义(P>0.05)。Cox回归分析显示,肝硬化、肿瘤直径是影响PLC患者总生存期的独立危险因素(均P=0.001)。结论 DSA引导下MWA结合DEB-TACE可提高PLC疗效,促进肝功能恢复,降低肿瘤标志物水平,改善预后。肝硬化、肿瘤直径与肝癌患者总生存期有关。
