摘要:
目的 探讨显著皮层静脉征(prominent cortical veins,PCV)对急性前循环脑梗死患者血管再通术后的影响。方法 纳入2020年1月1日至2022年12月21日中山市中医院急性前循环大动脉闭塞并且接受血管内再通治疗的脑梗死患者102例,收集临床和影像学数据。根据是否出现PCV,将患者分为PCV组和无PCV组,比较两组患者的临床资料。Logistic回归分析术后90 d mRS评分结局及与PCV发生的相关性。结果 与无PCV组相比,PCV组的心房颤动(房颤)发生率更高,侧支代偿良好率更低,术后(7±2)d的NIHSS评分更低,且差异均有统计学意义(P<0.05)。术后90 d,无PCV组mRS评分≤2分的比例为76.3%,高于PCV组的58.6%,但差异无统计学意义(P=0.074)。逻辑回归分析表明,PCV的存在与房颤呈正相关(OR=9.679,95%CI:1.264~74.364),与良好的侧支代偿呈负相关(OR=0.015,95%CI:0.033~0.331)。术后90 d良好功能预后与术前PCV的存在无关(OR=2.078,95%CI:0.680~6.348),而与年龄、术前静脉溶栓和术前ASPECT呈负相关(P<0.005)。结论 在前循环AIS患者中,PCV的发生与侧支代偿不良和心房颤动有关,但其对接受再灌注治疗后临床结局的影响仍不确定。PCV可作为脑血管侧支代偿不足的潜在评价指标。
