摘要:
目的 探讨肿瘤负荷评分(tumor burden score,TBS)在肝动脉化疗栓塞(TACE)联合微波消融(microwave ablation,MWA)治疗早中期肝细胞癌(HCC)患者预后的预测价值。方法 回顾性分析2013年1月至2019年12月79例在南通大学附属南通第三医院介入科接受TACE联合MWA治疗的HCC患者临床资料。采用时间依赖的ROC曲线及曲线下面积(area under the ROC curve,AUC)评价TBS(TBS2=肿瘤最大径2+肿瘤数目2)在5年生存率和无瘤生存率中预测的准确性。根据K-Means法选取TBS最佳截断值,将HCC患者分成3组作生存分析。Cox多因素分析筛选出影响接受TACE联合MWA治疗的HCC患者预后的独立因素。结果 ROC曲线统计显示TBS预测HCC患者经TACE联合MWA治疗后5年生存率的AUC最优,为0.815,肿瘤最大直径的AUC为0.684,肿瘤数目的AUC为0.640。根据最佳截断值,将79例HCC患者分成3组:TBS≤1.9( n =34,43.1%); 1.9<TBS≤3.3( n =25,31.6%); TBS>3.3( n =20,25.3%)。作生存分析显示,TBS≤1.9组的5年总生存率为76.5%,1.9<TBS≤3.3组56.7%,TBS>3.3组6.3%,随着TBS值的升高,患者的5年生存率逐渐下降。3组之间5年生存率比较差异具有统计学意义( P <0.05)。3组间无瘤生存率比较差异也具有统计学意义( P <0.05),且随着TBS值的升高,无瘤生存率逐步降低。多因素分析显示,TBS是影响HCC患者接受TACE联合MWA治疗后的总生存率和无瘤生存率的独立因素。结论 TBS对预测接受TACE联合MWA治疗的HCC患者的预后具有较好的效能。
