摘要:
目的 探讨经皮椎体成形术(PVP)+射频消融术(RFA)术后联合与不联合立体定向放疗(SBRT)缓解肺腺癌脊柱转移瘤患者疼痛的疗效对比,并分析治疗中影响疼痛的因素。方法 回顾性分析2020年1月至2024 年1月江苏省肿瘤医院收治的114例肺腺癌脊柱转移瘤患者的临床资料,其中接受PVP+RFA治疗53例,接受PVP+RFA+SBRT治疗61例,倾向性匹配后两组患者各有46例。比较两组患者治疗6个月后NRS评分和两组患者△NRS评分。单因素分析总结可能影响患者疼痛的因素,并随后将这些因素纳入二元logistic回归分析,寻找影响患者疼痛的独立影响因素。结果 PVP+RFA+SBRT组NRS评分明显低于PVP+RFA组,△PVP+RFA+SBRT组NRS评分明显高于△PVP+RFA组( P <0.05)。脊柱不稳定评分≤9、使用地舒单抗、使用唑来酸钠能够在治疗中明显缓解疼痛,是疼痛缓解的独立影响因素。结论 在PVP+RFA的基础上联合SBRT治疗,能够明显地减轻患者的疼痛。且脊柱不稳定评分≤9、使用地舒单抗、使用唑来酸钠的肺腺癌脊柱转移患者能够在治疗中获得更好的疼痛缓解效果。
