【摘要】目的评价X线引导下腰脊神经后内侧支松解治疗老年小关节源性腰痛的临床疗效。方法选取2017年1月至2018年12月武汉市第一医院疼痛科收治的老年小关节源性腰痛患者102例，随机均分成松解组和保守组，松解组采用X线引导下腰脊神经后内侧支松解治疗，保守组采用镇痛药物配合理疗。评价治疗前、治疗后1周及1、3、6、12、24个月两组腰痛评分和腰椎功能改善情况。结果松解组视觉模拟疼痛评分（visual analogue scores，VAS）在治疗后1周及1、3、6、12、24个月时均显著降低，差异有统计学意义（P＜0.05），且显著低于保守治疗组，差异有统计学意义（P＜0.05）。松解组腰椎功能障碍调查表得分、Oswestry功能障碍指数在治疗后1周及1、3、6、12、24个月时均显著降低，差异有统计学意义（P＜0.05），且显著低于保守治疗组（P＜0.05）。松解组和保守组均无相关并发症发生。结论X线引导下腰椎脊神经后内侧支松解治疗老年小关节源性腰痛安全可行，近期、中远期疗效显著。
